The project involves creating a whole-scheme evaluative system to be used across all the UK member hospitals. The money has come from the 2013-14 IESD Fund, under the Strategic Development programme, and was awarded to promote personalisation and choice of care and support. As with all the scheme’s innovations, the development process will be collaborative throughout so that the resultant system works for everyone.
